THE OPP VULNERABLE SECTOR CHECK CLINICS WILL BE HELD ON
TUESDAY OCTOBER 24, 2017 - 4PM – 8PM
WEDNESDAY OCTOBER 25, 2017 – 4PM-8PM

YOU MUST HAVE THE FOLLOWING DOCUMENTS WITH YOU IN ORDER TO BE PROCESSED AT THE CLINIC. PLEASE CONTACT THE SCHOOL SO WE MAY PROVIDE YOU WITH THE PROPER DOCUMENTATION.
(1) Letter requesting a Vulnerable Sector Check from St. Peter The Apostle
(I have these in the office, please contact me)
(2) A Vulnerable Sector Check form LE220E
You must have 2 pieces of ID please check the back of this page to see what qualifies as acceptable forms of ID.
Health cards and SIN Cards are not acceptable.
Also, if you have already submitted your record check you can still attend the clinic. We will pull your submitted record check and complete it while you wait. Please bring your photo ID.
